For New Patients: In order to establish a new patient, the parent or guardian with legal custody will need to bring the child(ren) to the first appointment. We will need your photo ID and insurance card(s).You can either print your forms here and complete before your appointment or if you prefer, you can come to your appointment 10-15 minutes early and fill out the forms in the office.Please bring the patient's immunization records, medical records (if requested) and any pertinent court documentation.
